Filling Out Your Program of Study

You will be prompted to login with your K-State eid and password to access this form. It may be accessed again when you later return to the form.

Full-time students should file their programs before the end of their second semester of graduate study when they have found a research advisor who is willing to take them on as a GRA.

To find the on-line form, go to https://ksu.etrieve.cloud/#/form/130/?focus=true .

Program Information

Select the type of degree you are pursuing from the drop-down menu and insert Physics as the degree program and Arts & Sciences in the College section.

KSU Credits

Please list all K-State courses and research credits (if applicable) and the semester taken. You can list a future date for courses not yet taken. You may list a single entry for research credits taken over multiple semesters with the total hours required and “semester taken” can be listed as “multiple”.

Kim can provide you with a spreadsheet if you’d like in order to assist you in completing the credits with your research advisor prior to entering the information in the online form so that everything is calculated properly.

Listed below is an example of how to enter the information:

Course #: PHYS 999

Course Title: Research in Physics (use title from the Course Catalog)

Semester Taken: Multiple

Credits: 52

Click on the "Add Course" button for each new course you add.

All core courses and Journal Club must appear on a PhD program of study. However, if you quizzed out of a core course you do NOT list that core course.

Total credit hours (classes and research) must add up to 90 hours for a Ph.D or 30 hours for an MS.

Don’t worry if courses change as you progress through your career as you can complete a "Program Change form".

Transfer Credits

Consult with Gary Wysin if you want to transfer any credits. After confirming you are eligible to transfer credits, you will list courses taken at other universities approved by your supervisory committee for your K-State program of study. You must provide course number, course title and semester taken as listed on the transfer university transcript and the name of the university.

If you are transferring credit from an awarded degree, you can list the course number as “degree”, the course title as “name of degree awarded” (ex. master of science) and for “semester taken”, please list the semester the degree was awarded. Your letter of admission to Graduate School indicates if you were eligible to transfer an awarded degree.

Supervisory Committee

You will choose a supervisory committee which consists of your major professor (or co-major professor) plus two other faculty in physics and one faculty outside of the physics department. Your major professor should provide guidance to you on these selections.

Enter the committee members first and last names, email address, department and select their role from a drop down menu. External committee members from outside of K-State must have received permission from the Graduate School prior to listing them. Consult Gary Wysin or Kim Coy for information on this process.

For faculty e-mail addresses on this form, use @k-state.edu not @phys.ksu.edu (e.g. wysin@k-state.edu). You can search for their eid at https://search.k-state.edu/?qt=people+search.
